What Information and Forms are Required When Applying for a Medical Marijuana in LA?

The following are acceptable documents:
1. Copy of driver’s license 2. State ID 3. Vehicle registration 4. Voter’s registration 5. Correspondence from another state agency with current date or a current car insurance bill.

- Completed online patient intake form - A completed and signed minor form for patients under 18 years of age with a designated custodial partner or legal guardian as the primary caregiver or authorized purchaser. - A completed and signed patient authorization form for applying caregivers with the patient’s license number which the patient will receive once their application is approved.
